The rapper had a medical emergency on stage on August 30.
Videos went viral of the rapper hyping up the crowd shortly before he collapsed.
Some concertgoers and paramedics rushed onto the stage to try to help the artist, giving him CPR, before he was taken to hospital.
His family and tour manager Birch Michael later confirmed he had died.
“It is with profound sadness and very heavy hearts that we share news of the passing of the legendary and iconic FatMan Scoop,” the family said in a post on his Instagram.
“Last night, the world lost a radiant soul, a beacon of light on the stage and in life.
“FatMan Scoop was not just a world-class performer, he was a father, brother, uncle and friend. He was the laughter in our lives, a constant source of support, unwavering strength, and courage.”
On his own Instagram post, Michael said: “It is with the heaviest of hearts I announce the passing of Isaac Freeman III, known professionally as Fatman Scoop.
“You taught me how to be the man I am today. I love you Scoop, thank you so much for everything you gave to me.”
Born Isaac Freeman III, Fatman Scoop was from New York City’s Harlem neighbourhood and broke out with 1999’s Be Faithful.
What started as a minor success in the US took off in Europe with a 2003 re-release, hitting No. 1 on the singles charts in the UK and Ireland.
The next year, he appeared on the UK television series Chancers, in which musicians mentored artists who wanted to make it in the US, the BBC reported.
He also was a contestant on Celebrity Big Brother 16: UK vs USA, which was filmed in the UK and aired in 2015.
Scoop was perhaps best known for his featured role on Elliott’s Lose Control, a 2005 song of the summer that also featured Ciara. The track won a short-form music video Grammy at the 2006 award show.
The same year as Lose Control, he was featured on Mariah Carey’s It’s Like That.
He also was featured on tracks from Timbaland, David Guetta, The Situation and Skrillex, among other artists.
In 2018, he reunited with Elliott and Ciara for a remix of the latter’s Level Up.
